The Frazer Center, with the Dekalb County Board of Health and The
American Kidney Fund, is sponsoring a HEALTH FAIR

Friday, June 23
10am - 3pm
Frazer Center-Atrium, 1815 Ponce De Leon Ave. NE, 30307

Screenings include:
  * Blood Pressure, Ear, Eye exam
  * Get your child's EED forms completed here for $25.00
  * Breast Examination Model
  * Cholesterol, diabetic screening, liver, heart and kidney testing

Refreshments will be provided!

Blue Cross/Blue Shield, The American Kidney Fund, Midtown Place
